Job Qulifications:
High School or general education diploma (GED)
One (1) year direct in-home care or related experience with a minimum of six (6) months direct in-home care experience.
Must possess effective verbal, written, and communication skills and adhere to designated deadlines.
Must possess the ability to assess and assist with resolving complaints or other service related issues.
Must have reliable transportation, valid drivers license and auto insurance; able to drive distances oer the branch service area.
Duties:
Provide direct in-home services to clients on a regular, fill-in, and/or emergnency basis, as assigned by Service Coordinator staff.
Conduct Home Care Aide and client monitoring visits in the home, to ensure compliance with the client care plans, on an intermittent or as needed basis, as directed by the Service Coordinator staff.
Complete and submit all required in-home visit forms to Service Coordinator staff in an accurate, legible, and timely manner.
Observe, document and report to Service Coordinator staff, any changes in a clients' condition that are noted during a home visit. Changes include but are not limited to: Physical, mental, emotional, financial, enviromental, medical, addistional service needs, etc.
Perform in-person follow-up on reported complaints or service related issues, as directed by Service Coordinator staff.
Deliver and review Client Welcome Packet and conduct in-home visits with new clients as requested.
Assist with in-home support and training of HCA's as needed.
Adhere to and maintain all required contractual and company policies and procedures.
Attend all required trainings per Addus and contractual requirements, as well as, attend all required meetings as directed by Agency Director.
Other duties as assigned.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
